Workplace Cardiovascular Risk Awareness

The goal of this program is to offer a first line of defense in the recognition, prevention, and rehabilitation of heart disease. With this in mind we offer industry the opportunity to further commit to their employee's quality of life by offering Cardiovascular Risk Awareness in the workplace.

Cardiovascular Risk Awareness includes cholesterol testing and blood pressure measurement, complimented by a risk factor review and a body mass index calculation based on height and weight. Many industries take this opportunity to augment the testing with one on one counseling with our cardiovascular nurse and/or dietitian on the Cardiovascular Risk Awareness day.

The concept of a Workplace Cardiovascular Risk Awareness Program, which focuses on not only education of the risk factors for heart disease but provides follow-up in the form of cardiovascular assessment and referral to appropriate programs, is unique to Niagara. Many valuable programs are implemented in workplaces, however, most of these programs are limited and do not provide any follow-up services to support ongoing risk factor reduction for the employees identified to be "at risk".


The services to assist people in recognizing their cardiovascular health risks include:

  1. Workplace Environmental Assessment
  2. Awareness & Education
    Encounter: Initial contact point
    Location: Workplace
    • Total Cholesterol (TC) to Good Cholesterol (HDL) ratio {mospagebreak
    • Risk stratification
    • Risk factor review
    • CVD indicator review (Peripheral Occlusive Arterial Disease, Stroke and Coronary Artery Disease)
    • Awareness/education package and counseling
    • Laboratory requisition provided if high risk score or high TC/HDL ratio
  3. Assessment & Follow-up
    Encounter: 2nd contact point
    Location: Workplace
          Low and moderate risk group
    • Counseling and targeting of specific elevated risk factor if any
    • Discuss Cardiovasuclar Risk Management Program or self directed home program
    • Report sent to family physician
       
          High and very high risk group
    • Laboratory results reviewed
    • Counseling and targeting of specific elevated risk factors
    • Enroll in Cardiovascular Risk Management Program
    • Report sent to family physician
       
          Patients with CVD symptoms identified
    • Referral to family doctor (FD) for further diagnostic testing

Heart Niagara Inc.'s (HNI) mandate is to participate at multiple levels of cardiovascular health services. In the Workplace Cardiovascular Risk Awareness Program, HNI is in a position to refer employees with newly recognized health concerns and conditions. In conjunction with the referral to the employee's family doctor, services are offered by HNI to help the individual manage their Cardiovascular Risk. The services are concentrated in one program, the Cardiovascular Risk Management Program.
